我國丙肝新葯丹諾瑞韋的研究近況如何？

丹諾瑞韋(Danoprevir)是由中國歌禮生物科技有限公司研製的一種非共價結合大環羧肽類蛋白酶抑製劑。在體外研究中，丹諾瑞韋對基因1、4、5、6型丙肝病毒都有較強的抑制作用[1-4]；在臨床研究中，丹諾瑞韋對基因1型和4型丙肝病毒感染者有良好的療效。

在我國的一項Ⅱ期臨床研究中，69例基因1型、初治非肝硬化的丙肝病毒感染者使用丹諾瑞韋聯合聚乙二醇干擾素及利巴韋林治療12周(MAKALU研究)，停葯12周的持續病毒學應答率達到96%。安全性良好，與丹諾瑞韋相關的不良事件只有噁心和腹瀉，丹諾瑞韋聯合聚乙二醇干擾素及利巴韋林3聯方案的不良反應與聚乙二醇干擾素+利巴韋林2聯方案相似，少數病人有輕度ALT和AST異常。3例病人(4%)停葯后複發。在我國台灣的一項研究中，基因1型肝硬化患者使用丹諾瑞韋聯合干擾素及利巴韋林治療24周，停葯12周的持續病毒學應答率達91%[5]。少數病人可發生耐葯，常見的耐藥性變異位點位於NS3蛋白酶R155和D168部位[6]。

丹諾瑞韋與利托那韋有相互作用，利托那韋可抑制丹諾瑞韋藥物代謝的酶系統，使藥物代謝減慢，提高丹諾瑞韋的療效。丹諾瑞韋+干擾素+利巴韋林+利托那韋4聯藥物治療初治丙肝患者24周，持續病毒學應答24可達96.6%，4型丙肝持續病毒學應答24高達100%[7]。我國台灣基因1b型非肝硬化丙肝病毒感染者使用這種4聯方案治療12周，停葯后的持續病毒學應答率達100%[5]。
